Ultrasound of the Week # 3 Case History

"Chest pain bounceback..."

A 54 year old AA female with history of HTN, hyperlipidemia, and resolved breast cancer presented to the CPU with worsening substernal chest pain over the last 4 days. She had been evaluated and admitted at an outlying hospital approximately a week before, and was unclear of her discharge diagnosis. She stated that she received an echo during her admission and was discharged on Indocin, but she was unable to tolerate this med secondary to severe GI upset. She discontinued Indocin 5 days prior to her Richland CPU admission.

She described her pain location as substernal, "pressure-like" in character, and without radiation. Her symptoms were improved while standing up. She had a temperature in the CPU of 100.3F, loud friction rub at the left lower sternal border, clear lungs and no heart murmur. Equal pulses in 4 extremities were noted along with absence of edema in the legs. No JVD was observed.
