Ultrasound of the Week # 2 Case History

"The case of the enlarged shoulder..."



A 59 year old African-american male with multiple medical problems presented to the ED after dialysis treatment with the chief complaint of severe shoulder pain, neck pain and headache. The patient explained that he had been cleaning his undercarriage the night before, slipped, and fell onto his left lateral neck and shoulder. He noted a profound deformity to the shoulder after the fall, which over the previous six hours was worsening.He denied loss of consciousness, shooting pains to the hands, vision changes, nausea or vomiting. He stated that he was unable to move his shoulder secondary to pain.

A preliminary shoulder xray series was ordered to rule out fracture or dislocation, seen below.
